You are responsible for reporting any unexpected changes in your medical condition to the health care provider. |

You are responsible for telling your doctor if you believe you can't follow through with your treatment. |

You are responsible for being considerate of the rights of other patients and personnel and assisting in controlling noise and number of visitors. |

You are responsible for respecting hospital property. |

You are responsible for keeping appointments and if unable to keep them, to notify the health care provider as promptly as possible. |

You are responsible for your own actions if you refuse treatment or do not follow the health care provider's instructions. |

You are responsible for providing information for insurance and for working with the hospital to arrange payment, when needed. |

You are responsible for any financial obligations related to health care and for seeing to it that they are taken care of as promptly as possible. |

You are responsible for adhering to hospital policies and procedures for safety and security, including No Smoking. |

You are responsible for signing a release of liability acknowledging the consequences of discharging against medical advice (AMA) when applicable. |

You are responsible for making complaints and constructive criticism known to hospital management. |

You are responsible for recognizing the effect of life-style on your personal health. |
